One packet includes your client information, health history, massage preferences, consent, and cancellation policy.

Complete & Sign
Type your answers in the PDF or fill it out by hand. Print and sign the forms, and mark your focus areas on the body diagram.

Bring It With You
Give your completed packet directly to Crystal at your appointment. Keep health details in your forms or discuss them in person.
Paper forms are also available at your appointment. Please allow extra time to complete them before your session.

Massage Consent & Scope of Practice
The consent form in your packet explains the scope of massage therapy, disclosure of health conditions, and your consent to receive massage. Read it before signing.
Massage is provided for relaxation and relief of muscular tension. It is not a substitute for medical examination, diagnosis, or treatment. Massage therapists do not diagnose medical conditions or prescribe medication.
Keep Crystal informed of health changes, current treatments, and any allergies. You may communicate your comfort level, pressure preferences, and areas to avoid at any time during your session.
The full consent and health-disclosure statements are included in the intake packet.
Cancellation Policy
Please give at least 24 hours' notice when canceling or rescheduling your appointment so the time can be offered to another client.
A fee applies to cancellations or rescheduling within 24 hours. Please ask Crystal to confirm the applicable fee before booking. A missed appointment without notice is charged at the full service cost.
Late arrival may shorten your session, with the full booked service cost still due. If you arrive more than 15 minutes late and there is not enough time to perform the service, the full service cost is charged.
